What is Complex PTSD?

Complex Post-Traumatic Stress Disorder (C-PTSD) is a mental health condition that develops after experiencing prolonged, repeated trauma, particularly during formative years or in situations where escape feels impossible.

Unlike single-incident PTSD, Complex PTSD results from ongoing traumatic experiences such as childhood abuse, domestic violence, long-term captivity, or extended periods of trauma.

C-PTSD affects not just how you respond to reminders of trauma, but fundamentally impacts your sense of self, your ability to regulate emotions, and how you relate to others. The condition creates profound disruptions in identity, self-worth, and interpersonal functioning that extend far beyond typical PTSD symptoms.

PTSD vs Complex PTSD

While PTSD and Complex PTSD share some common features, they are distinct conditions with important differences:

Traditional PTSD typically develops after a single traumatic event or short-term trauma exposure.

Symptoms include intrusive memories, flashbacks, avoidance of trauma reminders, hypervigilance, and changes in mood and thinking. PTSD often responds well to standard trauma-focused therapies.

Complex PTSD includes all the symptoms of PTSD, plus additional challenges related to emotional regulation, self-perception, and relationships.

These additional symptoms stem from prolonged trauma exposure and include difficulty controlling emotions, persistent negative beliefs about oneself, chronic feelings of shame or guilt, difficulty maintaining relationships, and a pervasive sense of disconnection from others.

The key distinction lies in the nature and duration of the trauma, as well as the broader impact on personality, identity, and social functioning.

You are not alone. You deserve to get help.

Arbor Wellness is an industry leader in mental health treatment. Our team of top medical experts specialize in dual diagnosis treatment and are committed to ensuring that each patient is treated as an individual. Call us today, we’re available 24/7.

Signs & Symptoms of Complex PTSD

Complex PTSD manifests through a wide range of symptoms across multiple domains of functioning:

Re-experiencing and Intrusion

Re-experiencing and Intrusion symptoms include flashbacks that feel overwhelming and all-consuming, nightmares that disrupt sleep, intrusive thoughts or memories that appear without warning, and intense emotional or physical reactions to trauma reminders.

Avoidance and Emotional Numbing

Avoidance and Emotional Numbing show up as deliberate avoidance of people, places, or situations that trigger memories, emotional numbness or detachment from feelings, difficulty experiencing positive emotions, and a sense of living in a fog or feeling disconnected from reality.

Emotional Dysregulation

Emotional Dysregulation creates persistent challenges with managing emotions, including intense anger or rage that feels uncontrollable, overwhelming sadness or depression, extreme anxiety or panic, rapid mood swings, and difficulty calming down once upset.

Negative Self-Perception

Negative self-perception involves chronic feelings of worthlessness or inadequacy, pervasive shame and guilt, viewing oneself as fundamentally broken or damaged, difficulty recognizing personal strengths or accomplishments, and a harsh, critical inner voice.

Interpersonal Difficulties

Interpersonal Difficulties manifest as challenges trusting others, fear of abandonment or rejection, difficulty maintaining close relationships, patterns of pushing people away or clinging too tightly, feeling fundamentally different from others, and struggles with boundaries.

Hypervigilance and Reactivity

Hypervigilance and Reactivity present as constant scanning for danger, being easily startled, difficulty sleeping or relaxing, feeling on edge or unable to feel safe, and overreacting to minor stressors.

Dissociation

Dissociation can range from mild spacing out to severe disconnection from thoughts, feelings, memories, or sense of identity, feeling as though watching yourself from outside your body, or losing time.

Residential Treatment for Complex PTSD

Residential treatment provides the most comprehensive level of care for Complex PTSD, offering 24/7 support in a structured, healing environment.

This intensive approach is particularly beneficial when outpatient treatment hasn’t been sufficient, symptoms significantly interfere with daily functioning, or you need a safe space away from triggering environments to focus on recovery.

At Arbor Wellness, our residential program provides a safe space where healing can occur at a deeper level. You’ll live on-site for the duration of treatment, typically ranging from 30 to 90 days, allowing for immersive therapeutic work without the distractions and stressors of everyday life.

The benefits of residential treatment for Complex PTSD include:

  • Intensive therapeutic support with multiple therapy sessions daily, allowing for rapid progress and the ability to work through trauma at your own pace.

  • A trauma-informed environment specifically designed to promote safety and healing. Every aspect of the program, from physical spaces to staff interactions, is structured to avoid re-traumatization and support your recovery journey.

  • Removal from triggering environments gives you space to heal without the constant reminders or stressors that may have kept you stuck.

  • Comprehensive, integrated treatment addresses all aspects of Complex PTSD simultaneously, including trauma processing, emotion regulation, relationship skills, and practical life skills. This holistic approach creates lasting change.

  • Peer support from others who understand the struggles of Complex PTSD reduces isolation and shame. The community aspect of residential treatment can be profoundly healing, offering connection in a way that may feel new and safe.

What Does a Typical Day Look Like In Residential Treatment?

At Arbor Wellness, each day is thoughtfully structured to provide intensive treatment while also allowing time for rest, reflection, and community connection.

Morning begins with a gentle wake-up and nutritious breakfast, followed by a mindfulness or meditation session to ground you for the day ahead. Mid-morning includes your first individual or group therapy session, where you’ll engage in trauma processing work or skills development.

Midday offers a healthy lunch prepared on-site, followed by free time for rest, journaling, or connecting with peers. Early afternoon features experiential therapy sessions such as art therapy, movement therapy, or time in nature, providing alternative avenues for processing and healing.

Afternoon includes additional group therapy focused on specific skill-building, whether that’s DBT skills, trauma education, or relationship development. You may also have individual therapy sessions with your primary therapist, where deeper trauma work occurs in a safe, one-on-one setting.

Evening begins with dinner as a community, fostering connection and practicing healthy social engagement.

Night includes a structured wind-down routine with support from staff to ensure you feel safe and settled. Sleep hygiene is emphasized, and support is available throughout the night if you experience distress.

Throughout the day, clinical staff remain available for support, check-ins, and crisis intervention if needed. The schedule balances structure with flexibility, allowing for spontaneous therapeutic moments and ensuring that the intensity of treatment doesn’t lead to overwhelm.

Weekends maintain therapeutic programming while incorporating more leisure activities, family contact opportunities, and outings when clinically appropriate.

How is Complex PTSD Treated?

Arbor Wellness Mental Health employs evidence-based and holistic therapies specifically proven effective for treating Complex PTSD:

Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)

Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) helps you process traumatic memories and change unhelpful thought patterns that developed as a result of trauma. Through TF-CBT, you’ll learn to identify and challenge distorted beliefs about yourself, others, and the world, gradually reducing the power trauma holds over your present life.

Dialectical Behavioral Therapy (DBT)

Dialectical Behavior Therapy (DBT) teaches crucial skills for managing the emotional dysregulation common in Complex PTSD. You’ll learn mindfulness practices, distress tolerance techniques, emotion regulation strategies, and interpersonal effectiveness skills that help you navigate relationships and difficult situations more successfully.

Somatic Experiencing

Somatic Experiencing addresses the ways trauma becomes stored in the body. Through gentle body awareness practices, you’ll learn to release trapped survival energy and restore your nervous system’s natural capacity for regulation. This body-based approach is essential since trauma impacts us physically as well as psychologically.

Psychiatry & Medication Management

Medication Management & psychiatry may be incorporated when appropriate to address dual diagnosis disorders such as depression, anxiety, or sleep disturbances. Our psychiatric team carefully evaluates each individual’s needs and collaborates with you to find the most helpful approach.
